Hey, it's been quite long: since the new year thing. Anyways, I think formmail.php could help you out. I'm not too sure whether it takes attachments in account, but you could use read about it and see whether it supports it. I use formmail a lot but it's one aspect (attachments) I've never explored with it: http://www.dtheatre.com/scripts/formmail.php

I did work on a website where the visitors (job seekers) should upload their resume (a file either MS word or PDF) via the form they were filling and when they clicked on "submit", it all comes to the project manager, that is the form and the submitted document. I believe what you need is something like that. I build the code myself, but I will have to dig up my code library and see whether I find it. If I do, I will let you know.
